This on-line form is used to report a discovery or invention made by faculty members, fellows, and staff members (including students on appointment as University employees), by anyone using University facilities, or by anyone collaborating with any of the above individuals ("Inventors"). Inventors at CU are bound by University of Colorado Policy on Discovery and Patents, per CU employee policy 5J.
The form consists of two sections, Inventors and Invention, both of which need to be filled out. Once you have completed both sections, you will have a chance to review (edit if necessary) and finish the process by clicking "Finish" button. You'll then receive a confirmation email including the submitted form as a pdf file.
Caution: Do NOT close browser windows during the process or all work will be lost.
